New clients must fill out a New Client Health History Form. It is a one page form and takes only a few minutes to fill out, but because it does ask questions about medications you may be taking and dates of your medical history many people prefer to complete it at home.

We’ve made that easy for you to do by providing the form here. Just click on the link below ~ print the form and fill it out at your leisure. If necessary, you may use the back of the form to give more information. Bring it with you on your first visit and we will discuss the information in order to design a program specifically for you.
